"Ultra-processed food" has become one of the most searched nutrition terms in the world - and one of the most confusing. Is bread ultra-processed? Is protein powder? Should you avoid anything with more than five ingredients?
This guide explains what ultra-processed foods (UPF) actually are, what the strongest research says about them - including a landmark review of almost 10 million people and two controlled feeding trials - and a practical way to spot UPF on the shelf without turning every grocery trip into a chemistry exam.
What "ultra-processed" actually means: the NOVA system
The term comes from NOVA, a classification developed by Prof. Carlos Monteiro's team at the University of São Paulo. NOVA sorts food not by nutrients, but by how much industrial processing it has been through:

What the research actually shows
1. The BMJ umbrella review: 32 linked health outcomes
In 2024, an umbrella review in The BMJ pooled 45 meta-analyses covering almost 10 million people. Higher UPF exposure was directly associated with 32 adverse health outcomes. The strongest ("convincing") evidence linked higher UPF intake to:
- About 50% higher risk of cardiovascular disease-related death

2. The NIH inpatient trial: +500 calories a day, on autopilot
In a tightly controlled NIH trial led by Kevin Hall, 20 adults lived in a research facility and ate ultra-processed and minimally processed menus matched for calories, sugar, fat, fiber, and macros - eating as much or as little as they wanted.
On the ultra-processed menu, people spontaneously ate about 500 more calories per day, ate faster, and gained roughly 1 kg in two weeks. On the minimally processed menu they lost the same amount. Same nutrients on paper - very different eating behavior.
3. The 2025 UCL trial: even "healthy" UPF underperforms
A 2025 crossover trial from University College London (published in Nature Medicine) went further: both diets followed official UK healthy-eating guidance, one built from minimally processed foods, one from ultra-processed products. After eight weeks on each, participants lost weight on both - but about twice as much on the minimally processed diet (2.1% vs 1.1% of body weight), without being told to eat less.
